The following is intended to be a guideline to help Liquid Trucking employees understand their new company health plan.

The new health plan goes into effect beginning in 2017.

We have switched employee health plans from a traditional Preferred Provider Organization (PPO) to a two-step health plan called Elite Choice High Performance Custom Provider Access. The intention behind the switch is to both save the company money, as well as give you the employee more flexibility, and more potential for out-of-pocket savings.

The first part of that plan is Elite Choice. Elite Choice is a Custom Provider Access program primarily aligned with Methodist Healthcare Systems and several other independent provider organizations in Omaha, Lincoln, and Council Bluffs. When you go to an Elite Choice provider, you will not encounter any balance billing issues. If you do choose to go to a provider that is not part of the Elite Choice network (which you may do) you may encounter some of these balance billing issues. That’s where the second part of the plan comes in.

The second part of the plan is Innovative Medical Risk Management (IMRM) Patient Advocacy & Provider Relations services. This company will step in and help you out if you find yourself in a situation where you have a balance billing issue. They will work with whatever parties are involved (you, your doctor, the hospital, etc) to resolve whatever issue might occur.

Some Common Questions:

Will my benefits change? Your benefits do not change, with the exception of one item. Using the emergency room for something that is not an emergency will result in a higher copay for you, so avoid that at all costs.

Can I still see the same doctor? You will still be able to see your same doctor. If your doctor has questions about your new plan, they should contact Mid-American Benefits.

What is a balance billing issue? If the provider you choose does not agree with the base level that we are going to pay for that service, it will have to be resolved

Does this change when my deductible resets? Your deductibles and coinsurance starts over on January 1st of each year, same as your previous program.
